Collection notice
This notice explains how LeadWise Connect handles personal information you give us through the forms on this website (contact, NDIS referral, aged care interest and feedback). It is a summary — our Privacy policy has the full details.
Who is collecting your information
LeadWise Connect Pty Ltd (ABN 49 702 892 519), trading as LeadWise Connect. Contact our Privacy Officer at admin@leadwiseconnect.com.au or 0418 789 939, or write to Level 1, 1 Palmerston Circuit, Palmerston NT 0830.
What we collect and why
- Contact form: your name, contact details and message, so we can respond to your enquiry.
- NDIS referral form: referrer and participant details, NDIS plan information, support needs, goals and risks (which may include health and disability information), so we can assess the referral and contact the participant about Support Coordination.
- Aged care interest form: your contact details and anything you tell us, so we can let you know about our aged care services.
- Feedback form: your feedback and, if you choose to give them, your contact details, so we can respond and improve our services. You can give feedback anonymously.
If you don't give us your information
Fields marked "required" are needed for us to respond. If you don't provide them, we may not be able to help, respond or process a referral.
Information about other people
If you give us information about someone else (for example, when making a referral), please make sure they know and have agreed, and let them know about this notice.
Who we may share it with
We only share your information with your consent, for purposes you would reasonably expect, or where the law requires or allows it — for example, with providers you ask us to connect you with, your plan manager or nominee, and our IT and hosting providers.
Overseas storage
Form submissions are processed and stored by our website host, Netlify, Inc., whose infrastructure is primarily located in the United States. Our website fonts are provided by Google, which receives technical information such as your IP address. Our email and IT providers may also store information outside Australia.
Access, correction and complaints
You can ask to access or correct your information, or make a privacy complaint, by contacting our Privacy Officer. Our Privacy policy explains how, and how to contact the Office of the Australian Information Commissioner if you're not satisfied.
