What is Support Coordination?
Support Coordination helps you understand and use your NDIS plan. A Support Coordinator works with you to find, connect with and coordinate the supports and services that help you work towards your goals — and helps build your own skills and confidence to manage your supports over time.
Support Coordination is funded through your NDIS plan when the National Disability Insurance Agency (NDIA) has included it. There are different levels, depending on how much help you need.
Level 2
Coordination of Supports
For participants who need help to understand their plan, choose providers, set up service agreements and bookings, and keep supports working together. Your Support Coordinator helps you build the skills to direct your own supports.
Level 3
Specialist Support Coordination
For participants whose situations are more complex — for example where there are higher risks, multiple systems involved (such as health, housing or justice), or barriers that need specialist skills to work through. The focus is on reducing complexity and building stability.
Who is it for?
- NDIS participants in the Northern Territory who have Support Coordination or Specialist Support Coordination funded in their plan, and whose Support Coordination funding is self-managed or plan-managed
- Families, carers and nominees supporting a participant
- Professionals and organisations referring a participant (with the participant's consent)
Who we can support
LeadWise Connect supports NDIS participants with self-managed or plan-managed funding for Support Coordination. We are not currently able to support NDIA-managed (agency-managed) plans.

How funding works
In general terms:
- Support Coordination is a capacity building support. If the NDIA includes it in your plan, it appears as a specific budget for Support Coordination.
- The amount of funding sets how many hours of support are available over your plan period, so we'll talk with you about how best to use them.
- Prices are charged in line with the NDIS Pricing Arrangements and Price Limits that apply at the time.
- Before we start, we'll agree on a service agreement with you that explains what we'll do and how it's charged.
- If your plan is plan-managed, we invoice your plan manager. If it's self-managed, we invoice you (or your nominee) and you pay from your NDIS funding.
